The Dr. Robert E. Appleby School Based Health Centers (SBHCs) provide on-site medical and mental health services to those who need it the most - children and adolescents - when they need it. The SBHCs are the only organization in Norwalk that provides no out of pocket cost for any of our services as well as the only agency in Norwalk that provides on-site school based medical and mental healthcare. The SBHCs are licensed as outpatient clinics (Public Health Code Sections 19-12DA45 through 19-13-53). The purpose of SBHCs is to provide a model of service delivery that ensures comprehensive care to children and adolescents - improving the overall physical and emotional health of students in order to maximize their educational experiences and potential. The SBHCs serve students (grades 9th - 12th) in the three Norwalk high schools: Norwalk High School, Brien McMahon High School and Briggs High School; as well as, students (grades 6th-8th) who attend Nathan Hale Middle School.

THE PRIMARY GOALS OF THE DR. ROBERT E. APPLEBY SCHOOL BASED HEALTH CENTERS INCLUDE:

Provide diagnosis and treatment of acute illnesses and injuries.

Provide preventative services, including outreach, family support, and health education.

Assist adolescents in adopting behaviors that promote their good health and well-being.

Assist adolescents through early identification of health issues and problems.

Provide a student centered, culturally defined, family focused, community-based model of healthcare.

Improve access to primary healthcare, psychiatric services and crisis intervention.

Provide school and/or sports physicals to those who are uninsured and/or underinsured.

Provide programs on wellness, disease prevention, and general health promotion.

Create Individual Healthcare Plans.

SERVICES INCLUDE:

School/sports physicals

Diagnosis and treatment of acute and chronic illnesses (i.e. strep, bronchitis, ear infections, asthma, etc.)

Substance abuse evaluation

Smoking cessation

Immunizations

Testing and treatment

Nutrition counseling and weight management

Individual, family and group therapy, as well as onsite psychiatric evaluations
